  1. Left hemisphere
usually dominant, controls language acquisition, math and reasoning
  2. Right hemisphere
    artistic and emotional
  3. Cerebrum
left control right, right controls left; initiation of movement, endocrine, autonomic functions
  4. Cerebellum
right controls right; uses info from cerebrum, muscles, joints and inner ear to coordinate movement, balance and posture
  5. Circle of Willis
    Internal carotid and vertebrobasilar arteries
  6. Stroke-third leading cause of death and leading cause of disability
  7. Risk factors
AFA males, people 51-74, family history
  8. Common factors leading to stroke
    Hypertension, cardiac disease, DM
  9. TIA
temporary neurologic deficit caused by impairment of cerebral blood flow; blood flow occluded by spasms, fragments of plaque, or blood clots
  10. Stroke
abrupt impairment of brain functioning resulting in a set of neurologic signs and symptoms that are caused by impaired blood flow to the brain and last for more than 24 hours
  11. Hemorrhagic
sudden onset, blood vessel in brain ruptures
  12. Intracerebral stroke
bleed in the cerebrum and usually associated with trauma, uncontrolled HTN and aneurysms
  13. Ischemic
Obstruction of blood vessel by plaque, blood clot, or both
  14. Embolic
originate from heart and circulate to cerebral vasculature; associated with AF, valvular heart disease and thrombi in left ventricle
  15. Thrombotic
caused by plaque in blood vessels in the brain; results in ischemia and infarction
  16. Hemorrhagic s/s
worst headache ever, LOC, vomiting and seizures
  17. Embolic s/s
one sided weakness, numbness, visual problems, memory lapses, headache, dysphagia
  18. Left sided damage
right side paralysis, aphasia, slow cautious behavior, memory deficit, aware of deficits
  19. Right sided damage
left side paralysis, spatial perceptual deficits, quick and impulsive behavior, memory deficits, impaired judgment, short attention span, left side neglect
  20. Dysarthria
inability to speak clearly due to control of muscles used in speech
  21. Dyspraxia
partial inability to initiate coordinated voluntary motor acts (inability to perform a previously learned action)
  22. Hemiplegia
    first flaccid and then spastic
  23. Initial diagnostic test for a stroke
CT to determine ischemic or hemorrhagic
  24. Lumbar puncture is not done is ICP is suspected
  25. Interventions directed at minimizing complications of a stroke
    hypertension, oxygenation, hyperthermia and hyperglycemia
  26. What does nimodipine do
calcium channel blocker decreases spasms in cerebral vessels after bleed
  27. Completed stroke
when neurologic deficits do not change for two to three days
  28. Receptive aphasia
difficulty understanding spoken or written words
  29. Expressive aphasia
    difficulty speaking and writing
  30. Fluent aphasia
sounds normal but makes little sense
  31. Nonfluent aphasia
    difficulty initiating speech
  32. Global aphasia
person has almost no ability to communicate
  33. Homonymous hemianopsia
perceptual problem that involves loss of one side of the field of vision
  34. What may be given in patient with acute strokes to dissolve clots
    tPA
  35. NIH stroke scale
    lower is better
  36. Ptosis
    drooping upper eyelid
  37. Used to treat cerebral edema
    Mannitol
  38. Used to treat hemorrhagic stroke by dilating and preventing spasms min cerebral blood vessels
    Nimodipine
  39. Used to reduce ICP by reducing cerebral inflammation
    corticosteroids
  40. Used to prevent strokes caused by thrombi
    aspirin
  41. Used to dissolve clots that cause acute ischemic stroke
    tPA
  42. Used to destroy thombi
    Streptokinase
