Mnemonic to assess mental state and/or level of consciousness.
- A-Alert. Communicates clearly w/out any stimuli
- V-Verbal. Responds to verbal stimuli
- P-Painful. Responds to painful stimuli
- U-Unresponsive. Doesn't respond to verbal and painful stimuli
