  1. Heparin
    A sulfated mucopolysaccharide; binds to antithrombin III and also has some direct inhibitory effect on Factor X. Can cause bleeding; thrombocytopenia; and osteoporosis stuff. Give it IV or SC; monitor; can reverse with protamine sulfate.
  2. Low Molecular Weight Heparin
    Fragments of Heparin; Weaker activity on AT III but still inhibits Factor X; fewer bleeding side effects; give SC once a day. Preferred over Heparin.
  3. Leprudin and Bivalirudin
    Direct thrombin inhibitors. Use in unstable angina
  4. Warfarin
    Vitamin K analogue; Blocks stuff of glutamate residues in clotting cascade proteins by keeping vitamin k from converting to its active form. Give it orally; reverse with vitamin k or factor IX stuff. Use for A fib or prosthetic heart valves. Teratogenic; so use LMW heparin instead if pregnant.
  5. Ximelagartan
    Orally active direct thrombin inhibitor. Prodrug; metabolized independent of P450. No drug interactions and doesn�t require monitoring of clotting time. Not in use yet.
  6. Streptokinase
    Fibrolytic agent; activates plasminogen to convert it to plasmin. Use for multiple pulmonary emboli; DVT; and acute MI
  7. Anistreplase
    Fibrolytic agent; Complex of plasminogen and streptokinase. Acetylated to protect active site. Gets infused into plasma; protecting group comes off and releases active part. Use for multiple pulmonary emboli; DVT; and acute MI
  8. Tissue Plasminogen Activator
    Fibrolytic agent; protease that preferentially hits plasminogen bound to fibrin. In theory; limits fibrinolysis to clots. Use for multiple pulmonary emboli; DVT; and acute MI
  9. Aspirin
    Antiplatelet agent; prevents synthesis of thromboxane A2 and prevents platelet aggregation. Irreversible COX 1 inhibitor. Use it for angina and coronary bypass surgery to reduce incidence of thrombotic graft closure.
  10. Ticlopine
    Antiplatelet agent; Inhibits ADP mediated platelet aggregation; use if you can�t tolerate aspirin; but not as good
  11. Clopidogrel
    Antiplatelet agent; Inhibits ADP mediated platelet aggregation; use if you can�t tolerate aspirin; but not as good
  12. Abciximab
    Antiplatelet agent; targets platelet receptors for integrin and other aggregating substances; mouse human antibody that blocks platelet receptors.
  13. Integrelin
    Antiplatelet agent; targets platelet receptors for integrin and other aggregating substances
