  1. Crust
    Dry blood, Serum, puss left on the skin surface when vessicles/ pustules burst large crust adhere to skin surface= scales
  2. Ulcer
    Deep, irregularly shaped area of skin loss extendiry into dermis, subcutaneous tissue may bleed, may leave a scar. 
  3. fissure 
    • linear crack with sharp edges extending into the dermis. 
    • ex.) mouth cracks, athlets foots
  4. scar
    • flat, irregular area of connective tissue left after a lesion, wand healed.
    •   new: red/purple
    •   old: white/ silver
  5. keloid
    Elevated  irregular darkened area of excess scartissue caused by excessive collagen formed during healing. Extends Beyond the site of original injury. ( more can decent)
  6. Excoriation
    • Linear Erosion
    •   Ex.) scratches, chem burn

    Nurse must asses, interpret all of these not UAP( not done by (UAP)
