  1. Functional Class
    Class IV antidysrhythmic (Calcium channel blocker)
  2. Mechanism of Action
    • Inhibits calcium ion influx through slow channels into the myocardial cells and arterial smooth muscle
    • Causes intracellular calcium to remain at sub-threshhold levels
    • Reduces peripheral vascular resistance by inhibiting the arterial smooth muscle
    • Inhibits coronary artery spasm
    • Slows SA and AV node conduction without effecting atrial action potential
  3. Indications
    • SVT refractory to adenosine
    • Rapid A-Fib/A-Flutter
  4. Contraindications
    • Hypersensitivity
    • Sick sinus syndrome
    • 2nd and 3rd degree AV block
    • Hypotension
    • WPW
    • Wide complex tachycardia
  5. Precautions
    • Avoid in patients on oral beta blockers
    • Monitor for hypotension resulting from peripheral vasodilation
    • Use with caution in CHF patients
  6. Side Effects
    • Chest pain
    • Bradycardia
    • Hypotension
    • Syncope
    • CHF
    • Dysrhythmias
    • N/V
    • Headache
    • Sweating
    • Dizzness
    • Dyspnea
    • 2nd and 3rd degree AV block
    • Palpitations
  7. Interactions
    Beta blockers
  8. Dose and Route
    • Bolus:
    • 0.25 mg/kg slow IVP, may repeat in 15 mins at 0.35 mg/kg slow IVP

    • Infusion:
    • 5-15 mg/hr, titrate to heart rate
