  1. magnesium sulfate (mgso4)

    tocholytic drug
    -depresses myometrium contractility, relaxes smooth muscle of the uterus.

    maternal: flushing, drowsiness, headache, lethargy, nausea, vomiting

    no effects on neonate

    monitor FHR and UC's, serum magnesium levels
  2. indomethacin naproxen and fenoprofen

    prostaglandin synthesis inhibitors

    tocolytic drug
    depresses synthesis of prostaglandins, effective in delaying delivery 48 hours because of fetal side effects generally used short term and before 32 weeks

    maternal: GI upset, serious side effects are uncommon

    fetal effects: SERIOUS FETAL SIDE EFFECTS, constriction of the patent ductus arteriosis, neonatal pulmonary hypertension, oligohydramniosis, and intraventicular hemorrhages

    monitor FHR and UC's, treat nausea and heartburn
  3. Nifedipine and Nicardipine

    tocholytic drug
    inhibits smooth muscle contractions of uterus by blocking calcium availability for muscle contraction, delays delivery 48-72 hrs

    maternal: flushing, headache, nausea, transient tachycardia and hypotension

    fetal: may decrease uteroplacental blood flow

    monitor FHR and UC's, maternal BP and HR
  4. ritodrine and terbutaline

    tocholytic drug
    beta 2 adrengeric effects to suppress uterine activity, can delay delivery for 3 days

    maternal: tachycardia, cardiac arrhythmia, myocardial ischemia, and pulmonary edema, elevation in glucose, hypokalemia

    fetal: mild tachycardia, IVH

    monitor FHR and UC's, I&O for fluid overload, auscultate lungs, monitor HR, glucose
  5. methergine (methylergonovine)
    PPH due to uterine atony or subinvolution

    Action: stimulates contraction of the uterine smooth muscle

    side effects: nausea, vomiting, cramps

    check BP, this med can increase the BP
  6. carboprost tromethamine (hemabate)
    PPH that has not responded to oxytocin or metergine therapy

    action: uterine contractions

    side effects: diarrhea, nausea, vomiting, fever
  7. hydralzine (apresoline, neopreol)

    antihypertensive meds (1st line)
    • -vasodilator
    • -used for sever preeclampsia
    • -caution for rapid BP decrease
    • -can cause decrease uteroplacental perfusion and o2
  8. mehyldopa (aldomet)

    antihypertensive med (1st line)
    • -may work on CNS
    • -may take a few days for onset
    • -no research on long term effect on fetus
  9. labetalol (beta blocker)

    antihypertensive med (1st line)
    • -slows HR and decreases systemic vascular resistance
    • -no research on long term effect on fetus
  10. nifedapine

    antihypertensive med (2nd line)
    -calcium channel blocker (procardia) controls hypertension rapidly, increases cardiac index, increases urinary output
  11. morphine
    • -opioid
    • -CNS depression, neonatal resp. depression
    • -avoid close to delivery time (1 hr)
  12. meperdine (demerol)
    • -opioid
    • -CNS depressionl, neonatal resp. depression
    • -avoid use close to delivery time (1 hr)
  13. Butorphanol (stadol) and Nalbuphine (nubain)
    • -opioid agonist antagonist
    • -no resp. depression in woman or neonate
    • -check moms history for drug abuse
    • -dont give to drug dependent woman because mom and baby can experience withdrawal
  14. Sublimaze (fentanyl)
    • -short acting opioid antagonist
    • -crosses the placenta rapidly
    • -FHR changes, hypotension, maternal/fetal/neonatal CNS depression, resp. depression
    • -monitor for sedation, nausea, vomiting, itching, resp. rate
  15. promethazine (phenergan) and hydroxyzine (vistaril)
    • -ataractics
    • -drowsiness
    • -agonist effect of narcotics
  16. naloxone (narcan)
    • -used to reverse narcotic effects
    • -can be used to relieve maternal itching and resp. depression

  17. oxytocin (syntocinon)
    • -postpartum control of bleeding
    • -action: stimulates uterine smooth muscle to produce uterine contraction
    • -adverse reaction with IV use: coma, seizures, hypotension, water intoxication
  18. ibuprofen (motrin)
    • -mild to moderate pain
    • -action: decrease pain and inflammation
    • -side effects: headaches, constipation, dyspepsia, nausea, vomiting, edema
  19. docusate (colace)
    • -prevention of constipation
    • -action: promotes incorporation of water into the stool
    • -side effects: mold abdominal cramps
  20. tolterodine (detrol)
    • -action: inhibits cholinergic mediated bladder contractions
    • -side effects: dry mouth, headache, dizziness
  21. vit. k (phytonadione and aquamephyton)
    • -prevention of hemorrhagic disease in neonate
    • -action: vit. k is required for the hepatic synthesis of blood coagulation factors II, VII, IX, X
    • -side effects: erythema, pain, swelling at injection site
  22. erythromycin ophthalmic ointment
    • -prophylaxis treatment for honococcal or chlamydial eye infections
    • -action: prevents bacterial growth by inhibiting folic acid synthesis
    • -side effects: edema and inflammation of eyelids
    • -hold 1/2 inch from eye
