  1. Definition of Chorioamnionitis
    • Intraamniotic infection (IAI)
    • Inflammation of chorion/amnion
    • Typically multi-organism bacterial infection of AF, F membranes, placenta and/or uterus
  2. Diagnosis of Chorioamnionitis
    • fever >38C (100.4F)
    • maternal tachycardia (<>100-120)
    • fetal tachycardia (>160)
    • uterine tenderness
    • purulent/foul smelling AF
    • leukocytes (>15,000-18,000)
    • Gold Standard: amnio culture of AF - esp. for subclinical presenation
  3. Sequelae associated with Chorioamnionitis
    • M/N sepsis
    • neonatal pneumonia
    • PROM/PPROM
    • PTL/PTB
    • CP & other neurologic damage
    • Neonatal mortality (1-4% if PROM, >10% of PPROM)
  4. Management of Chorioamnionitis
    • Collaborate/refer if: preterm; term w/o response to abx. treatment; s/s of sepsis; worsening status; developing shock
    • Indication for BIRTH (not CS!): induce; broad spectrum Abx (ampicillin & gentamycin); acetaminophen; continuous EFM
    • Cord ABGs, placenta to pathology
