  1. CNS Infection or Meningeal Irritation
    • Signs are global, not focal¬†
    • Neck mobility
    • Kernig's sign
    • Brudzinski's sign
    • Irritability
    • Slowed mental function
    • Altered vital signs
    • Generalized weakness
  2. Neck Mobility
    • Pt positioned in supine, flex neck to chest
    • Positive sign = neck pain, with limitation and guarding of head flexion due to spasm of posterior neck muscles
    • Can result from meningeal inflammation, arthritis or neck injury
  3. Kernig's Sign
    • Pt positioned in supine
    • Flex hip and knee fully to chest then extend knee
    • Positive sign = causes pain and increased resistance to extending the knee due to spasm of hamstring
    • When bilateral, suggests meningeal irritation
  4. Brudzinski's Sign
    • Pt positioned supine, flex neck to chest
    • Positive sign = causes flexion of hips and knees (drawing up)¬†
    • Suggests meningeal irritation
  5. Slowed Mental Function
    • Examine for persistent HA, increased head-down position
    • May progress to delirium, lethargy, and coma
  6. Altered Vital Signs
    • Examine for:
    • Increased HR and RR
    • Fever
    • Fluctuating BP
