  1. KAWASAKI DISEASE:
    • a.k.a mucocutaneous lymph node syndrome
    • acute systemic inflammatory dse

  2. Cx:
    cardiac: aneurysm (an excessive localized enlargement of an artery caused by a weakening of the artery wall).
  3. Assessment:
    • 1. ACUTE:
    • -fever
    • -conjunctival hyperemia
    • -red throat
    • -swollen hands, rash and enlargement of the cervical LN

    • 2. SUBACUTE:
    • -crakcing lips and fissures
    • -desquamation of the skin on the tips of the fingers and nose
    • -joint pain
    • -cardiac manifestation
    • -thrombocytosis

    • 3. CONVALESCENT STAGE:
    • -child may appear normal + (+)signs of inflammation
  4. INT:
    • ~monitor TEMP frequently
    • ~assess: heart sound, rate and rhythm
    • ~strict I&O
    • ~give soft foods and liquids that are neither too hot or too cold
    • ~weigh daily
    • ~ACETYLSALICYLIC ACID (aspirin): antipyretic and antiplatelet effects
    • ~Immunoglobulin IV to reduce duration of fever and coronary artery lesions and aneurysm (ensure blood precaution)
  5. Parent education:
    • -essential follow-up care
    • -s/sx of kawasaki dse: irritability for 2 mos
    • -peeling: hands and feet
    • -pain in joints
    • -stiffness: AM, after naps, and in cold temp
    • -!HCP if temp is 38.3 Celsius (record temp until pt is afebrile for several days)
    • -aspirin may be prescribed
    • -WOF signs of ASPIRIN TOXICITY:
    • tinnitus
    • headache
    • vertigo
    • bruising
    • *DO NOT EXPOSE THE CHILD TO ASPIRIN IF THE CHILD HAS BEEN EXPOSED TO CHICKENPOX OR FLU.
    • -s/sx of bleeding:
    • epistaxis
    • hemoptysis
    • hematuria
    • melena
    • bruising
    • -s/sx of cardiac cx:
    • chest pain or tightness (older children)
    • cool and pale ext
    • abdominal pain
    • n&v
    • irritability
    • restelessness
    • uncontrollable crying
    • -avoid contact sports if child is taking aspirin or anticoagulants
    • -avoid MMR vaccines or varicella vaccine for 11 mos after IV immunoglobulin therapy
