  1. 10% Glucose Solution
    • Level I 
    • Adult & Peds: 5ml/kg IV Max
    • Single Dose 250ml


  2. Amiodarone
    • Level I
    • VF/Pulseless V-Tach: 300mg IV/IO, repeat dose 3-5 minutes 150mg
    • Peds: 5mg/kg IV/IO, repeat 3-5 minutes to a max dose of 15mg/kg
    • Tachycardia with Pulse: 150mg in 100ml NS IV/IO over 10 minutes
    • Repeat same dose to a Max of 450mg
  3. Lidocaine (Xylocaine)
    • Level I
    • VF/Pulseless V-Tach: 1-1.5mg/kg IV/IO for first dose, 0.5-0.75mg/kg every 5-10 minutes 
    • Max Dose: 3 Doses or 3mg/kg
    • Peds VF/Pulseless V-Tach: 1mg/kg, repeat X2 to max dose of 3mg/kg
  4. Magnesium Sulfate
    • Level II
    • Adult Torsades DE Pointes: 1-2G diluted in 100ml NS given over 5-20 minutes
    • Peds Torsades DE Pointes: 25-50mg/kg Max 2G diluted in 100ml NS given over 5-20 minutes
    • Adult Respiratory Distress: 1-2GM in 50ml NS over 5 minutes.
    • Peds Respiratory Distress: 40mg/kg max of 2G/30 min.
    • Pregnancy Seizure: 2-4G
  5. Epinephrine
    • Level I
    • Asystole/PEA: 1mg IV/IO every 3-5 minutes 
    • Bradycardia: Epi Drip 2-8 mcg/min
    • Peds VF/Pulseless V-Tach: 0.01mg/kg IV/IO every 3-5 minutes
    • Peds Asystole/PEA: 0.01mg/kg IV/IO every 3-5 minutes
    • Peds Bradycardia: 0.1-1mcg/kg/minIV drip with base order. Repeat every 3-5 minutes.
  6. Adenosine
    • Level I
    • 6mg Rapid IV Push, Repeat 12mg, Repeat 12mg
    • Peds: 0.1 mg/kg (max of 6mg)may repeat x 2 (max of 12mg)
  7. Atropine
    • Level I
    • Bradycardia: 0.5mg IV, repeat doses to a total of 3mg
    • Peds Bradycardia: 0.02 mg/kg (Minimum Dose 0.1mg)May repeat to max of 1mg.
    • Organophosphate Poisoning:
    • Adult: 2mg every 5 minutes as needed.
    • Peds: 0.05-0.1mg/kg every 5 minutes as needed.

  8. Dopamine
    • Level II
    • Bradycardia: Dopamine Drip 2-10mcg/kg/min
    • Peds: 2-20mcg/kg/min
  9. Glucagon
    • Level II
    • Greater than 8 YO: 1mg IM
    • Equal or less than 8 YO: 0.5mg IM
  10. Nitroglycerin
    • Level I
    • 0.4mg SL, repeat every 3-5 minutes as long as BP remains over 90
  11. Aspirin
    • Level I
    • 325mg to chew
  12. Fentanyl
    • Level I
    • Chest Pain: 50mcg slow IV/IM/IO/Atomized
    • Repeat 50mcg to a max of 200mcg

  13. Morphine Sulfate
    • Level I
    • Adult: 5mg, repeat to a max of 20mg
    • Peds: 0.1-0.2mg/kg to a max dose of 10mg.

  14. Naloxone (Narcan)
    • Level I
    • Adults: 0.4-2mg IV,Atomized, IM,IO
    • Peds: 0.1mg/kg IV,Atomized,IM,IO
  15. Epinephrine 1:1,000
    • Level I
    • Adult: 0.3mg IM
    • Peds: 0.01mg/kg IM
  16. Benadryl (Diphenhydramine)
    • Level II
    • Adult: 50-100mg IM or 25-50mg Slow IVP.
    • Peds: 12.5mg IM
  17. Versed (Midazolam)
    • Level II
    • Adults: 1mg IV, may repeat in 1mg increments to a max of 5mg.
    • Peds: 0.2mg/kg IV, may repeat in 0.2mg increments to a max of 5mg.
  18. Oral Glucose
    • Adult and Peds: 15 GM PO
  19. Odansetron (Zofran)
    • Level I
    • Adult: 4mg, may repeat x 2 every 10 minutes to a max of 12mg.
    • Peds: Above age 4, 4mg. Max dose of 4mg.
  20. Activated Charcoal
    • Level II
    • Adult: 50 GM PO
    • Peds: 50 GM PO
  21. Calcium Chloride
    • Level II
    • Adult: 1 GM
    • Peds: 20mg/kg
  22. Sodium Bicarb
    • Level II
    • Adult and Peds: 1meq/kg IV/IO
  23. Albuterol
    • Level I
    • 2.5mg in 3ml NS, may repeat as needed.
  24. Ipratropium Bromide (Atrovent)
    • Level I
    • 500mcg mixed with Albuterol. May repeat every 20 minutes to a max dose of 3 doses.
  25. Valium (Diazepam)
    • Level II
    • Adult: 5mg IV/IO. May repeat 1 x after 10 minutes.
    • Peds: 0.3mg/kg IV/IO to a max of 5mg. Rectal 0.5mg/kg to a max dose of 10mg.
