  1. Functional Class
    Sympathomimetic
  2. Mechanism of Action
    • Synthetic catecholamine
    • Acts primarily on Beta-1 and Alpha receptors
    • Increases cardiac output via increases inoptropic effects
    • Decreases Pulmonary Wedge Pressure (PWP)
    • Little or no effect on BP/HR
    • Also increases conduction through AV node
  3. Indications
    • Short term management of CHF or Cardiogenic shock
    • Increase cardiac output without increases rate
  4. Contraindications
    • Hypersensitivity
    • VT
  5. Precautions
    • Can increase myocardial oxygen demand
    • Can be deactivated by sodium bicarbonate
    • Preexisting HTN/a-fib
    • Hypotension from hypovolemia; volume restoration first
  6. Side Effects
    • Tachycardia
    • HTN
    • Dyspnea
    • Dysrhythmias
    • Palpitations
    • CP
    • Headaches
    • Anxiety
    • N/V
  7. Interactions
    • Absolute interaction with beta blockers as the decrease dobutamines effectiveness
    • Use with caution with TCAs
  8. Dose and Route
    2-20 mcg/kg/min IV infusion
  9. Preparation
    250 mg/250 mL or 500 mg/500 mL NS or D5W = 1000 mcg/mL
  10. Note
    Dopamine is drug of choice in cardiogenic shock
