  1. Explain the Purpose of E-0, Rx Trip or SI
Provides actions to verify proper response of the automatic protection systems following manual or automatic actuation of Rx Trip or SI, to assess plant conditions, and to ID appropriate recovery procedures.
  2. Major Action Categories of E-0
    • -Verify automatic actions as initiated by protection and safeguards systems
    • -ID appropriate recovery procedures
    • -Shutdown unnecessary equipment and continue trying to ID appropriate recovery procedure
  3. Entry conditions to E-0
    • Rx Trip required as determined by setpoints or requirements being exceeded.
    • Rx Trip has occured as determined by annunciators, power, DRPI, RBLs.
    • Rx Trip & SI required as determined by setpointds or requirements being exceeded.
    • Rx Trip & SI has occured as determined by annunciators, SI Pump status, or other means.

    • Containment Pressure High-1 2/3 3.5psig
  6. Immediate Action Steps of E-0
    • 1. Check Rx Trip-RBLs on-RTB open-PWR lowering.
    • If not, manually trip, goto FR-S.1 if >=5% or(+) IR SUR

    • 2. Check Turbine Trip-All Stop valves closed.
    • If not manually trip turbine. Close MSIVs if won't trip

    • 3. Check power to AC Emerg Buses a) @least one b) both
    • If neither powered, goto ECA0.0, if one not, start EDG close breaker.

    • 4 Check SI actuated, and both trains
    • If SI didn't actuate, check if needed. if not goto ES-0.1. else actuate. if only one actuate, turn SI switches.
  7. Continuous action steps of E-0
    13 Check RCS 557F

    19 Check SG levels

26 Check if RHR should be stopped

A8 Check if containment spray needed.
  8. Addenda associated with E-0
    • Foldout ADD19 Aligning ESW to AFW suction
    • 10 ADD18 AFW Emergency Alignment
    • 25 ADD11 Restoring SG Sampling after SI
    • 27a ADD7 Restoring Offsite Power
    • 27b ADD8 Loading Equipment on AC Emergency Buses
    • 27c ADD9 Placing EDGs in Standby
    • A10 ADD26 SIS Statuds Panel Alignment
    • A11 ADD25 Containment Isolation Phase A Valves
    • A13 ADD27 CRIVS Status Panel Alignment
  9. Foldout Page Criteria
    • 1. RCP Trip Criteria
    • If both of the following occur, Trip the RCPs
    • a) any CCP or SIP running
    • b) RCS pressure<1475psig

    • 2. Faulted SG Criteria
    • If any SG pressure lowers uncontrollably or is completely depressurized perform the following:
    • a) Fast Close MSIvs
    • b) Isolate Feed flow to faulted SG
    • c) Manually close or locally isolate failed ASDs
    • d) Maintain total Aux Feed flow > 355,000lbm/hr until at least one SG level is >7%

    • 3. Ruptured SG Isolation Criteria
    • If both of the following conditins occur, isolate Feed to the affected SG
    • a) Level in any SG rises uncontrollably
    • b) NR lavel in affected SG >7%{25%}

    • 4. Cold Leg Recirculation Criteria
    • If RWST level <36%, then perform ES1.3 Transfer to Cold Leg Recircualtion

    • 5. AFW Supply Switchover Criteria
    • If CST to AFP Suction pressure is <2.75psig, then perform ADD19 Aligning ESW to AFW Suction
  10. Should seal injection be secured if the RCP is stopped.
    Explain
Seal Injection should be maintained to all RCPs

Seal injection should be maintained to keep seals cool and clean.
  11. Which pressure transmitters should be used to determine if any SG is faulted and why.
The ESFAS SG transmitters may be innaccurate if a secondary line break occurs in Area 5. the pressure indicators on the SG ASD controllers are not affected and should be used for comparision.

The ASD pressure transmitters are located in the AFW room, and are physically isolated from the Aux Building side of Area 5
  12. Adverse Containment conditions
Containment pressure >=3.5psig (recovarable by pressure lowering to <3.5psig)

Containment Radiaiton >=105 R/hr (requires an evaluation to resume normal setpoints)
  13. When is Critical Safety Function Monitoring initiated?
When Transitioning out of E-0 or when Step 18 of E-0 is reached.
  14. Transitions from E-0
    • Foldout page CL Recirc, goto ES1.3
    • 1.RNO Rx did not trip and Power >5% or IR SUR is positive, goto FR-S.1
    • 3. Power to NB01 nad NB02 lost, goto ECA0.0
    • 4. If SI not actuated and not required, goto ES0.1
    • 11. Check if PORVS & Spray closed. If PORVs and Block cannot be closed, goto E-1
    • 14 Faulted SG, goto E-2
    • 15. Ruptured SG, goto E-3
