  1. Functional Class
    Atropine: parasympatholytic

    Pralidoxime chloride: cholinesterase reactivator
  2. Mechanism of Action
    • Atropine counters parasympathetic response from muscarinic receptor over-stimulation associated with organophosphate and nerve agent poisoning
    • Pralidoxime chloride reactivates acetylcholinesterase by binding to it and subsequently releasing the phosphoryl group associated with the poisoning
  3. Indications
    Organophosphate and nerve agent poisonings in MCIs
  4. Contraindications
    None in emergency setting
  5. Precautions
    • No prophylactic protection, only administer if s/s present
    • Do not treat fasiculations caused by nicotinic receptor over-stimulation
  6. Side Effects
    • Dry mouth
    • Flushing
    • Pupil dilation
    • Tachycardia
  7. Dosage and Route
    • Contains:
    • Atropine 2 mg
    • Pralidoxime chloride 600 mg

    Minor symptoms initially: One Mark 1 Kit autoinjector

    If severe symptoms set in within 10 mins of first dose: Two Mark 1 Kits autoinjectors

    • If severe symptoms present initially:
    • Three Mark 1 Kits
  8. Note
    Tube one (atropine) is always administered first

    Diazepam is drug of choice for fasiculations
