Hints for Iron Deficiency Anemia:
- Microcytosis and hypochromia
- Elevated RDW
- Decreased retic count
- Elevated platelet count
Lab patterns of Iron Deficiency Anemia?
- Fe low
- TIBC high
- % saturation low
- Ferritin low
- Soluble Transferrin Receptor Assay should be elevated with iron-deficiency states
How much Fe do you need to replete stores in Fe-deficiency anemia?
Need 200-250 mg of elemental iron daily to replete stores until serum Fe normalizes, and once H/H normalize, replace for 6 more months.
Labs in Anemia of Chronic Disease:
- Serum Fe: low
- TIBC: low
- Ferritin: high
- % saturation: low/normal
- Soluble transferrin receptor: Normal
- Bone marrow assay: increased iron stores
