    Calcitriol

    2mg/2ml per port

    400mg

    300-325mg QHS or 1 hr b/4 dialysis
  15. Vancomycin
    given intraperitoneal- 30mg/kg LD in long dwell, then 15mg/kg in long dwell every 3-5days)
  16. Cefazolin
    given intraperitoneal 15mg/kg QD per exchange
  17. Calcium Carbonate
    1-2g PO TID /w meals
  18. Calcium Acetate (phoslo)
    2 tabs PO TID /w meals
  19. Sevelamer (Renagel)
    800-1600mg TID /w meals
  20. Lanthanum Carbonate (Fosrenol)
    250-500mg TID /w meals
