  1. Ambulation
    • 1. Greet resident, address by name, introduce self
    • 2. Tell them your going to go for a walk with them, is now a good time?
    • 3. Give them call button, close curtain, wash hands
    • 4. Apply gait belt around waist, carefully, snug, not too tight
    • 5. Support patients shoulder and wrist while helping them up (1..2..3..up)
    • 6. Ask if theres any dizziness or discomfort upon standing
    • 7. Walk slightly behind & to one side of resident while holding gait belt in back
    • 8. Walk resident 10 steps forward 10 steps back
    • 9. Ask resident about comfort, dizziness, or fatigue during ambulation
    • 10. Assist resident to turn & position to sit back in chair (when back of legs touch chair, have them sit)
    • 11. Provide controlled gentle lowering of resident into chair
    • 12. Remove gait belt carefully, not pulling or sliding off but gently lifting off

  2. Bedpan
    • *Supplies: gloves, bedpan, moist towellete, chuck, toilet paper, 1 barrier towel
    • 1. Greet resident, address by name, introduce
    • 2. Is now a good time to assist with your bedpan?
    • 3. Give call button, close curtain, wash hands
    • 4. Ask permission to get supplies, put on gloves
    • 5. Unfold barrier towel on table, put bedpan & toilet paper on towel
    • 6. Get chuck, ask patient to put their hands to their sides, pull sheet back on one side, ask to bend knees and lift up
    • 7. Place chuck on bed under them
    • 8. Position bedpan under them
    • 9. Raise bed in sitting position (60-90 degrees)
    • 10. Give patient call button & toilet paper, tell them you'll be back in a few minutes to check on them
    • 11. Lower bed before removing bedpan, ask if they want a moist towellete for hands
    • 12. Ask resident to life up again, remove bedpan & chuck @ same time, wrapping the chuck around the bedpan. Dispose of barrier towel
    • 13. Give call button
    • 14. Turn on water in bathroom
    • 15. Empty bedpan contents into toilet
    • 16. Rinse & dump 3 times
    • 17. Pat dry & disinfect
    • 18. Place barrier paper towel on counter, set bedpan on
    • 19. Remove gloves & wash hands
    • 20. Grab bedpan with paper towel
    • 21. Ask permission to put away supplies

  3. Catheter Care- Indwelling Urinary Catheter
    • *Supplies: gloves, 2 washcloths, 2 towels (barrier, med), basin, soap, chuck
    • 1. Greet resident, address by name, introduce self
    • 2. Explain you'll be performing catheter care, is now a good time?
    • 3. Give call button, close curtain, wash hands
    • 4. ASk permission to get supplies, put on gloves. Put barrier towel on table, put all supplies on there
    • 5. If there's not already a chuck under patient have them place hands to their sides, bend knees, & lift up, place chuck under them
    • 6. Fill basin with about 2 in. of warm water
    • 7. Test with elbow, ask patient to test
    • 8. Put washcloth in to wet it
    • 9. Lower blankets down, put medium towel across stomach for barrier
    • 10. Apply soap to washcloth, lather, fold into taco
    • 11. Change spot on soapy washcloth for each washing stroke, wipe down one side of labia, down the otherside & across the top. Use the last spot to clean cath. tube
    • 12. Check that the tubes aren't tangled
    • 13. Hold tubing @ top & swipe down 4 inches
    • 14. Hang soiled soapy towel on side of basin
    • 15. Use 2cd washcloth, soak in water
    • 16. Use different spot for each rinsing stroke
    • 17. Rinse same spots (areas)
    • 18. Pat dry
    • 19. Adjust gown & bedsheets
    • 20. Avoid tugging or pulling on catheter
    • 21. Leave tubing free of kinks or obstructions & not touching the floor
    • 22. Remove old chuck, pushing through to the other side, NEVER pulling it toward you
    • 23. Put new chuck under resident give call button
    • 24. Dispose of soiled linens & old chuck
    • 25. Turn on water in bathroom, dump basin in toilet (rinse & dump 3 times) flush
    • 26. Pat basin dry, disinfect
    • 27. Put barrier paper towel down, place basin on towel
    • 28. Remove gloves, wash hands
    • 29. Get barrier towel to grab basin, ask permission to put it away, open curtain
  4. Change Occupied Bed
    • *Supplies: 1 barrier towel, top sheet, bottom sheet, pillowcase
    • 1. Greet resident, introduce self, address by name
    • 2. Explain that your here to change their sheets, is now a good time?
    • 3. Give call button
    • 4. Close curtain, wash hands
    • 5. Ask permission to get supplies
    • 6. If head of bed is raised, lower it
    • 7. Raise whole bed up
    • 8. Position resident safely on non working side of bed
    • 9. Loosen tucked dirty sheet (bottom) on working side & push or roll sheet & tuck it against the resident, under body
    • 10. Place clean bottom sheet on working side, securing under mattress @ head of bed & along working side
    • 11. Extend clean bottom sheet on working side, across bed & tuck under resident
    • 12. Assist resident to turn to face opposite side of bed
    • 13. complete placement of bottom sheet, securing flat sheet @ head of bed & on side (using fitted sheet, secure all four corners)
    • 14. Do a 6 point tuck, pull & tuck 3 times on each side to prevent creases & bed sores. Move patient back to a back lying position
    • 15. Put clean top sheet on top of dirty sheet
    • 16. have patient hold onto the clean sheet & pull the dirty one out
    • 17. Tuck in top sheet using hospital corners
    • 18. Bunch up pillowcase
    • 19. Put pillow long way up & put pillowcase on, grab top of pillow & pull case on without shaking the pillow or putting the pillow on your body
    • 20. Dispose of dirty linens
    • 21. Pull once @ the toes so feet aren't restricted

  5. Change Position to a Supported Side Lying Postition
    • *Supplies: 1 barrier towel, 3 pillows
    • 1. Greet resident by name, introduce self
    • 2. Let them know your going to change their position, is now a good time?
    • 3. Give call button, close curtain, wash hands, ask to get supplies
    • 4. Supporting shoulder & knees (underneath) move patient to side of bed
    • 5. Have them cross their arms & legs @ ankles & roll them gently to their side
    • 6. Place a pillow under the shoulder/back, tuck under, fold & tuck again, have them lay back a little on the pillow
    • 7. Place pillow in between knees, don't let feet touch
    • 8. Place pillow under armpit & under chest. Don't let their hands touch. Keep patient covered
    • 9. Pllace call button in their hand
    • 10. Let them know you'll be back in 2 hours to change position again
    • 11. Open curtain, wash hands
  6. Dress a Resident who has a weak arm
    • *Supplies: 1 barrier towel, 2 shirts, 2 pants, socks
    • 1. Greet resident by name, introduce self
    • 2. Explain that your going to dress them, is now a good time?
    • 3. Give call button, close curtain, wash hands
    • 4. Ask if you can get supplies
    • 5. Put down barrier towels
    • 6. Place clothes on top of towel
    • 7. ASk resident which shirt & which pants they would prefer
    • 8. Remove gown (good arm out first)
    • 9. Dress injured arm 1st, put on sleeve by bunching it up gently & sliding onto arm, take rest of shirt & scrunch it under the back
    • 10. Go to the other side & gather the rest of the shirt, put it on arm same way, button ALL buttons
    • 11. Put on socks
    • 12. Put on pants (scrunch up pants & slide on both legs @ once
    • 13. Never expose patient
    • 14. Give call button, ask if they want the head of bed up or down
    • 15. Open curtain
    • 16. Wash hands
  7. Feeding a Patient
    • *Supplies: 2 towels (1 barrier 1 med), tray of food, washcloth, cup of water, towellettel.
    • 1. Greet resident by name, introduce self
    • 2. Explain that you'll be helping them eat, is now a good time?
    • 3. Give call button
    • 4. Close curtain, wash hands, ask permission to get supplies
    • 5. Raise head of bed 60-90 degrees so resident is sitting
    • 6. Ask resident if they want to wash their hands (give moist towellette)
    • 7. Put protective towel under chin
    • 8. Offer them a drink & then wipe their mouth
    • 9. Use spoon to feed NEVER fork
    • 10. Offer fluids to drink while feeding, after every 3-4 bites
    • 11. Wipe mouth & check to see if resident swallowed after every bite, wipe mouth
    • 12. Offer encouragement to resident toward maximizing food & fluid intake
    • 13. Converse with resident during the meal
    • 14. Remove & dispose of linen & trash give call button
    • 15. Open curtain, wash hands, record intake
  8. Foot Care
    • *Supplies: 1 basin, 1 towel, 1 chuck, 1 washcloth, soap, lotion
    • 1. Greet resident by name, introduce self
    • 2. Explain that you'll be doing foot care is now a good time?
    • 3. Give call button, close curtain, wash hands
    • 4. Ask patient for permission to get supplies
    • 5. Fill basin w/about 2 in of water. Check temp with elbow, ask patient to check
    • 6. Place chuck on floor, place water basin on chuck
    • 7. Remove resident's shoes & socks
    • 8. Place residents bare foot in the water to soak
    • 9. Place washcloth in basin to wet, add soap directly to washcloth
    • 10. Wash foot gently, top, bottom, sides, & between toes, rinse foot with washcloth
    • 11. Remove foot from water & dry completely. Pat dry top, bottom, sides, toes
    • 12. Ask if they would like lotion, warm lotion in hand first, don't put between toes
    • 13. Remove excess lotion with towel, put on socks & shoes
    • 14. Provide support to the ankle & calf
    • 15. Avoid placing bare foot on the floor before, during & after foot care

  9. Hair and Nail Care
    • *Supplies: 3 towels (1 barrier, 2 small), 1 basin, comb, nail stick, nail file, lotion

    • 2. Explain that your there to do hair & nail care, is now a good time?
    • 3. Give call button, close curtain, wash hands
    • 4. Ask resident if you can get supplies
    • 5. Put barrier towel down first, get everything else put down
    • 6. Give resident call button
    • 7. Fill basin with 2 in of water, test with elbow, ask resident to test
    • 8. Place residents hand in basin to soak
    • 9. Put towel over residents shoulders, ask if they want hair up or down
    • 10. Comb in small sections, beginning @ ends & working way up
    • 11. Pull comb through hair gently, throw comb away
    • 12. Dispose of towel that's around shoulders
    • 13. Take hand out of basin, pat dry
    • 14. Remove residue under nails with flat side of stick, swipe and wipe
    • 15. File nails down, down across, feel for jagged edges
    • 16. Ask if they want lotion, warm before applying. Pat off excess lotion with towel

    • 18. Dispose of barrier towel, take basin to bathroom, turn on water

  10. Measure & Record Blood Pressure
    • *Supplies: Blood pressure cuff, stethoscope, 2 alcohol pads
    • 1. Greet resident by name and introduce self
    • 2. Explain that your going to take their blood pressue, is now a good time?
    • 3. Give call button
    • 4. Close curtain, wash hands, ask to get supplies, clean stephoscope
    • 5. Expose residents antecubital
    • 6. Position the residents arm with palm up & elbow @ level with heart
    • 7. Wrap cuff evenly around arm with bottom of cuff positioned within an inch above antecubital
    • 8. Locate the brachial artery before placing the stethoscope
    • 9. Postition diaphragm of stethoscope over brachial artery & place stehoscope earpieces in before inflating cuff
    • 10. Inflate cuff safely. Pump to 160
    • 11. Control deflation of cuff
    • 12. First thump systolic
    • 13. Hear nothing is dystolic
    • 14. Clean stethescope again

    • 5. Open alcohol pad

    • 11. Tap tube with finger to get excess of (flick)

    • 13. Fold chuck over graduate & set aside, lower bed

    • 5. Place fingers over radial pulse
    • 6. Count pulse for one full minute
    • 7. Record pulse rate
    • 8. Tell resident your going to take their pulse again for accuracy, (but check their respiration)
    • 9. Count respirations for @ least one minute (each rise & fall of chest/stomach is one respiration)

    • 7. Ask patient if they have any cuts or sores
    • 8. Moisten toothbrush with water & apply toothpaste
    • 9. Brush all surfaces of teeth & gum line with a gentle motion (top)
    • 10. Offer resident opportunity to rinse mouth & spit into emesis basin, wipe mouth
    • 11. Ask if patient is feeling pain/discomfort
    • 12. Brush bottom
    • 13. Offer to spit, rinse & wipe mouth

    • 16. Dispose of protective towel on patient, throw away toothbrush

  15. Provide Mouth Care to Resident With Dentures
    • *Supplies: 2 towels (1 barrier, 1 small), paper towel, gloves, emesis basin, denture brush, toothette, dentures, cup of water, toothpaste
    • 1. Greet resident by name, introduce self
    • 2. Explain that you'll be providing mouth care, is now a good time?
    • 3. Give call button, close curtain, wash hands
    • 4. Ask permission to get supplies
    • 5. Put small towel on patient as barrier

    • 9. Rub top of mouth & gums with toothette

    • 11. Wet toothette again, gently clean bottom of mouth

    • 15. Remove barrier towel, ask to put away supplies
    • 16. Pour water in emesis cup, dispose of toothette, give call button
    • 17. Transport denture cup, denture brush, toothpaste, & emesis basin to sink
    • 18. Put down denture cup, denture brush & toothpaste
    • 19. Turn on water, dump emesis basin into sink, rinse & dump 3 times
    • 20. Pat dry, disinfect, place barrier towel down, place basin on towel
    • 21. Drain denture cup, rinse & dump 3 times, fill with about 2 inch water

    • 23. Line sink with paper towel to fill sink before you clean dentures

    • 28. Get a barrier towel & grab basin collect everything else with gloved hand

  16. Partial Bed Bath
    • *Supplies: gown, 2 towels (1 barrier, 1 med), basin, soap, lotion, washcloth
    • 1. Greet resident by name, introduce self
    • 2. Explain that your there to give a partial bed bath, is now a good time?
    • 3. Give call light, close curtain, wash hands
    • 4. Ask permission to get supplies
    • 5. Remove sleeves of gown & then cover resident back up, put new gown under pillow, give call light
    • 6. Fill up basin with 2 in of water, test temp with elbow, ask resident to check
    • 7. Put medium barrier towel over chest, under neck
    • 8. Wet wscloth, form a mitt so the corners aren't flapping around
    • 9. Wash eye without soap from inside out, changing to a clean area of the washcloth before cleaning other eye
    • 10. Wash each side of face in a half moon motion, use a different part of the washcloth for each side of face
    • 11. Pat dry, pull covers & gown down & place barrier towel over the sheets below the belly button
    • 12. Wash neck, using soap, one swipe on each side, rinse same way, pay dry
    • 13. Wash torso, down one side & up the other in circular motion
    • 14. Rinse the same way, in circular motions, pat dry, cover patient
    • 15. Expose just arms, wash arms down, then up, swipe armpit
    • 16. Rinse the same way, pat dry
    • 17. Cover patient, only move sheets to expose side
    • 18. Supporting shoulder & under knees, move patient to the side of bed, ask to cross arms & legs
    • 19. Roll patient to the side, position barrier towel on the bed by back so water doesn't roll down
    • 20. Wash back of neck one swipe each side
    • 21. Rinse same way, pat dry
    • 22. Ask resident if they would like lotion
    • 23. Warm lotion in hands & massage in circular motion, moving up, pat off excess
    • 24. Cover resident & roll back over
    • 25. Put new gown on, tie @ neck
    • 26. Ask if you can put away soap & lotion
    • 27. Dispose of soiled towels, give call button
    • 28. Turn on water, dump basin in sink, rinse & dump 3 times
    • 29. Pat dry, disinfect
    • 30. Put barrier towel down, put basin on it
    • 31. Remove gloves, wash hands
    • 32. Get another barrier towel to grab basin
    • 33. Ask permission to put basin away, make sure they have call button, open curtain
  17. Perineal Care (Female)
    • *Supplies: 2 chucks, 2 towels (1 barrier, 1 med), 2 washcloths, gloves, basin, soap
    • 1. Greet resident by name, introduce self
    • 2. Explain you'll be doing perineal care, is now a good time?
    • 3. Give call button, close curtain, wash hands, put on gloves
    • 4. Ask permission to get supplies
    • 5. Remove soiled chuck (push through to toher side, never pull toward you)
    • 6. Replace with a new chuck
    • 7. Fill basin with about 2 in water, check temp with elbow, ask resident to check
    • 8. Soak 1 washcloth in water
    • 9. Apply soap, lather, fold into taco
    • 10. Move bedsheets down, place barrier towel over stomach
    • 11. Wipe labia (wipe down) both sides & swipe across top, use different spot on washcloth for each swipe, place dirty washcloth over side of basin
    • 12. Get second washcloth for rinsing, soak in water, fold into taco
    • 13. Rinse same area you just washed pat dry, cover resident
    • 14. Supporting shoulder & under knees move resident to side of bed
    • 15. ASk them to cross their arms & ankles & roll them onto their side
    • 16. Lower sheets & place barrier towel under bottom so sheets don't get wet
    • 17. Wash butt area, wipe butt cheek from inner to outer, wipe top of butt
    • 18. Rinse same area, pat dry, roll over to back
    • 19. Remove chuck & place a new one under resident
    • 20. Cover, give call button
    • 21. Ask permission to put away soap, dispose of soiled linens
    • 22. Dump basin (into toilet) while water is running, rinse & dump 3 times, flush
    • 23. Pat dry, disinfect
    • 24. Put barrier towel down, put basin on it
    • 25. Remove gloves wash hands
    • 26. Grab barrier towel to grab basin, throw other towel away
    • 27. Ask permission to put basin away
    • 28. Give resident call button, open curtain
  18. Range Of Motion: Lower Extremity
    • 1. Greet resident by name, and introduce self, explain you'll be doing ROM exercises on lower body, is now a good time?
    • 2. Give call button, close curtain, wash hands
    • 3. Support lower extremity, above & below joints (ankle & underneath knee)
    • 4. Take residents hip & knee through ROM, flexing knee & hip raising towards torso, returning back to mattress 3 times
    • 5. Take residents hip through abduction/adduction ROM exercises, support ankle & under knee & gently pull leg off bed to side 3 times
    • 6. Support ankle & under knee & rotate leg toward you 3 times
    • 7. Lift leg & flex ankle 3 times
    • 8. Rotate ankle 3 times
    • 9. Flex toes 3 times
    • 10. Make sure patient doesn't feel any discomfort
    • 11. Give call button, close curtain, wash hands
  19. Range Of Motion: Upper Extremity
    • 1. Greet resident by name & introduce self, let patient know you'll be doing ROM on arms (upper body) is now a good time?
    • 2. Give call button, close curtain, wash hands
    • 3. Support extremity, above & below joints
    • 4. Take shoulder through ROM raising & lowering straightened arm alongside towards head of bed & back to mattress 3 times
    • 5. Take resident's shoulder through abduction/adduction ROM extend arm out to the side & back 3 times
    • 6. Take shoulder through rotation ROM, put elbow on bed & bend arm back by head & then back to mattress 3 times
    • 7. Provide ROM to wrist, flex, rotate & extend fingers & thumb 3 times
    • 8. Make sure resident doesn't feel any discomfort
    • 9. Give call button, close curtain, wash hands
  20. Transfer Resident From the Bed into a Wheelchari using a transfer belt & pivot transfer technique
    • *Supplies: wheelchair, transfer belt, non skid shoes
    • 1. Greet resident by name & introduce self
    • 2. Explain you'll be transferring them to a wheetlchair, is now a good time?
    • 3. Give call button, close curtain, wash hands
    • 4. Get gait belt, put on seat of wheelchair
    • 5. Place wheelchair @ the side of bed near end (foot) swing footrests out of the way
    • 6. Move sheet without exposing resident, only exposing side
    • 7. With support of shoulders & under knees, move patient to the side of the bed in sitting position
    • 8. Put on non skid shoes
    • 9. Without moving away from patient, move wheelchair @ a close angle to the patient, lock wheels, turn small wheels out like duck feet
    • 10. Apply gait belt snugly, ensure residents feet are flat on the floor
    • 11. Stand in front of resident, each knee on the outside of patients closed legs
    • 12. Have patient put their arms around neck, hold gait belt on the sides
    • 13. Help patient into wheelchair using pivot method
    • 14. Unlock wheels, move patient away from bed
    • 15. Lock wheelchair
    • 16. Stand behind wheelchair, ask them to cross their arms, hold their elbows & gently move them back into wheelchair
    • 17. Support ankle & swing footrests into place
    • 18. Move wheelchair to final position & lock wheelchair again
    • 19. Remove gait belt
    • 20. Give call button, open curtain, wash hands
  21. Wash Hands
    • 1. Stick 2 fingers under water
    • 2. Run one hand @ a time under the water up to wrist
    • 3. Apply soap
    • 4. lather front & back of hands, between fingers, around cuticles, under nails & wrists
    • 5. Provide cleansing friction for @ least 15 seconds
    • 6. Rinse always hold fingers lower than your wrists
    • 7. Dry each hand with a separate paper towel, wiping up
    • 8. Use a 3rd towel to turn off faucet
