  1. Cyclosporine
    • immunosuppressant
    • bind cyclophilin-->inhibit calcineurin--> inhibit IL-2 production--> block Tcell activation

    • USE- suppress organ rejection after transplant
    • -autoimmune disorders

    • SE- nephrotoxic! (prevent with mannitol diuresis)
    • -increase risk of viral infections and lymphoma
  2. Tacrolimus
    • immunosuppressant
    • bind FK-binding protein--> inhibit IL-2 secretion

    USE- suppress organ rejection after transplant

    • SE-Nephrotoxic! peripheral neuropathy, HTN
    • Pleural effusion, hyperglycemia!
  3. Sirolimus (rapamycin)
    • immunosuppressant
    • bind, block mTOR--> inhibit T cell proliferation in response to IL-2

    USE- immunosuppressant after kidney transplant in combo w/ cyclosporine + corticosteroids

    SE- hyperlipidemia, thrombocytopenia, leukopenia
  4. Daclizumab
    • immunosuppressant
    • -monoclonal antibody with high affinity of IL-2 receptor on activated T cells

    USE- acute rejection of kidney transplant
  5. Azathioprine
    • immunosuppressant
    • antimetabolite precursor of 6-mercaptopurine--> block metabolism/synthesis of nucleic acids
    • -toxic to proliferating lymphocytes

    USE- kidney transplant, autoimmune disorders (glomerulonephritis, autoimmune hemolytic anemia)

    • SE- bone marrow suppression.
    • -metabolized by xanthine oxidase (so accumulates with Allopurinol!!)
  6. Muromonab-CD3
    • Immunosuppressant
    • -monoclonal antibody to CD-3 (epsilon chain)
    • --> block cell interaction with CD-3 for TCR signal

    USE- immunosuppressant for kidney transplant

    SE- Cytokine release syndrome! hypersensitivity reaction

    • USE- renal cell carcinoma
