  1. Single Engine Failure PROCEDURE:
    • Collective -- Reduce to maintain OEI limits.
    • Airspeed -- 70 KIAS.
    • RPM switch -- INCR; set NII RPM at 100%.
    • Throttle (affected engine) -- Closed.
    • Boost Pump switch (affected engine) -- OFF.
    • Fuel switch (affected engine) -- OFF.
    • Fuel XFEED switch -- OVRD CLOSE.
    • Fuel INTCON switch -- OPEN.
    • GEN switch (affected engine) -- OFF.
    • Battery BUS 1 & 2 -- ON.
    • MASTER CAUTION -- RESET.
  2. Possible reason to attempt engine restart in flight?
    If flameout was caused by a malfunction of the automatic mode of the fuel control unit.
  3. Engine restart in flight PROCEDURE:
    • Throttle (affected engine) -- Closed.
    • BOOST PUMP switch (affected engine) -- ON.
    • FUEL switch (affected engine) -- ON.
    • FUEL XFEED switch -- NORM.
    • GOV switch (affected engine) -- MANUAL.
    • GEN switch (affected engine) -- OFF.
    • For battery start (non-assisted):
    • BATTERY BUS switch (normal engine)-- OFF.
    • BATTERY BUS switch (affected engine)--ON.
    • START switch -- Engage.
    • Throttle -- Open slowly at 12% NI until ITT begins to rise. Do not open throttle further until ITT & NI RPM stabilize.
    • START switch -- Centered at 55% NI RPM.
    • Throttle -- Increase slowly; adjust as required to control torque, ITT, and N1 RPM.
    • NOTE -- Try to keep torque of affected engine 4% below torque of normal engine.
    • GEN switches (both) -- ON.
    • BATTERY BUS 2 switch -- ON.
    • FUEL TRANS switch (affected engine) -- ON.
    • FUEL INTCON switch -- NORM.
  4. Dual Engine Failure PROCEDURE:
    Collective pitch -- Reduce. Establish glide at 65 to 90 KIAS.

    • Best angle of glide = 90 KIAS
    • Minimum rate of descent = 65 KIAS

    Accomplish autorotative landing.

    If time permits before landing and a restart will not be attemped:

    • Throttles (both) -- Closed.
    • BOOST PUMP switches (both) -- OFF.
    • FUEL switches (both) -- OFF.
    • FUEL TRANS switches (both) -- OFF.
  5. Engine underspeed INDICATIONS:
    Low ENG RPM and ROTOR RPM (possibly with RPM warning light and audio if power demand is in excess of single engine power available).

    TORQUE split (proportional to power demand).

    Low GAS PROD RPM (N1), ITT, and TORQUE on affected engine.

    • Collective -- Adjust as necessary to maintain ROTOR RPM.
    • Airspeed -- 70 KIAS or as required to maintain flight within OEI limits.
    • Affected engine -- Identify.
    • Throttle frictions -- Tighten on normal engine; reduce on affected engine.
    • GOV switch (affected engine) -- MANUAL.
    • Throttle (affected engine) -- Increase slowly. Adjust throttle and collective as required to maintain TORQUE of affected engine slightly below TORQUE of normal engine.
    • MASTER CAUTION light -- Reset.
    • Land as soon as practical.
  7. Engine Overspeed -- Fuel Control/ Governor Failure INDICATIONS:
    • High ENG RPM (NII) and ROTOR RPM, possibly with RPM warning light.
    • Definite TORQUE split (proportional to power demand).
    • High GAS PROD RPM (NI), ITT, and TORQUE on affected engine.
    • Return of ENG RPM (NII) and ROTOR RPM to governed value (if power demand is high).
  8. Engine Overspeed -- Fuel Control/ Governor Failure PROCEDURE:
    • Collective -- Adjust as necessary to maintain ROTOR RPM. >>>>>TRIPLE TACH<<<<<
    • Airspeed -- 70 KIAS or as required to maintain flight within OEI limits.

    • Throttle (affected engine) -- Increase slowly. Adjust throttle and collective as required to maintain TORQUE of affected engine slightly below TORQUE of normal engine.
    • MASTER CAUTION -- Reset.
  9. Governor Actuator Failure (Full Increase) INDICATIONS:
    NII and rotor RPM increase to approximately 101%.

    RPM INCR-DECR switch inoperative.
  10. Governor Actuator Failure (Full Increase) PROCEDURE:
    If failure occurs during takeoff or landing, no immidiate corrective actions is necessary to complete either maneuver.

    As soon as practical, roll back both throttle to maintain 97 to 100% NII RPM.
  11. Engine Overspeed - Driveshaft Failure INDICATIONS:
    • Left yaw.
    • Rapid decrease in Nr.
    • Rapid increase in NII.
    • Illumination of rotor RPM warning light with audio.
    • Increase in noise due to:
    • Overspeeding engine turbines.
    • Overspeeding combining gearbox.
    • Driveshaft breakage.
  12. Engine Overspeed - Driveshaft Failure PROCEDURE:
    • Enter Autorotation.
    • Throttles -- Close, if time permits.
  13. Engine Compressor Stall INDICATIONS:
    • Engine 'pops'.
    • High or erratic ITT.
    • Decreasing or erratic NI and NII RPM.
    • Torque oscillations.
  14. Engine Compressor Stall PROCEDURE:
    • Hover:
    • Land as soon as possible.
    • In-flight:
    • Collective -- Reduce, maintain slow flight. Avoid large, rapid increases in collective pitch.
    • Heater switch -- Reposition.
    • ITT and NI -- Check for normal indications.
    • Land as soon as practical.
  15. Engine Hot Start/Shutdown INDICATIONS:
    Flames emitting from the exhaust.

    Excessive IT indication.
  16. Engine Hot Start/ Shutdown PROCEDURE:
    • Abort start of affected engine as follows:
    • Throttle -- Closed; keep starter engaged.
    • BOOST PUMP switch -- OFF.
    • FUEL switch -- OFF.
    • Starter -- Contintue to energize until ITT decreases.
    • Complete shutdown.
    • Exit helicopter and check for damage.
  17. Engine Oil Hot INDICATIONS:
    Engine oil temperature above limit of 120 degrees C.
  18. Engine Oil Hot PROCEDURE:
    • Collective, Airspeed -- As required.
    • Throttle (affected engine) -- Idle.
    • ENG switch -- INCR; set ENG RPM (NII) at 100% if possible.
    • If condition persists:
    • Throttle (affected engine) -- Close.
    • Complete engine shutdown.
    • Land as soon as possible.
  19. Engine Fire During Start PROCEDURE:
    • Abort start of affected engine as follows:
    • Throttle -- Closed.
    • FUEL XFEED switch -- OVRD CLOSE.
    • BOOST PUMP switch -- OFF.
    • FUEL switch -- OFF.
    • Appropriate FIRE handle -- Pull.
    • FIRE EXT switch -- MAIN.
    • If FIRE warning light remains on more than 10 seconds:
    • FIRE EXT switch -- RESERVE.
    • Complete engine shutdown.
    • Exit helicopter.
  20. Engine Fire During Takeoff or Approach PROCEDURE:
    • Airspeed -- 45 KIAS minimum.
    • Collective -- Reduce (altitude permitting).
    • Appropriate FIRE PULL handle -- Pull.
    • FIRE EXT switch -- MAIN.
    • If FIRE warning light remains on more than 10 seconds:
    • FIRE EXT switch -- RESERVE.
    • NII RPM -- Set at 100%.
    • Land as soon as possible.
    • Complete engine shutdown.
  21. Engine Fire In Flight PROCEDURE:
    • Initiate emergency descent immediately if possible.
    • Shut down affected engine as follows:
    • Approapriate FIRE PULL handle -- PULL.
    • Throttle (affected engine) -- Closed.
    • FIRE EXT switch -- MAIN.
    • FUEL XFEED switch -- OVRD CLOSE.
    • BOOST PUMP switch (affected engine) -- OFF.
    • FUEL switch (affected engine) -- OFF.
    • Fuel INTCON switch -- OPEN.
    • If FIRE warning light remains on:
    • FIRE EXT switch -- RESERVE.
    • NII RPM (normal engine) -- Set at 100%.
    • Land as soon as possible.
  22. Cabin Smoke or Fumes PROCEDURE:
    • VENT BLOWER switch -- ON.
    • Vents and windows -- Open.
    • If additional ventilation is required:
    • Airspeed -- Reduce to 60 KIAS or less.
    • Passenger doors -- Open.
    • Land as soon as possible.
  23. Baggage Compartment Fire PROCEDURE:
    • Reduce power to minimum required.
    • Land as soon as possible.
    • Inspect tailboom area for damage.
  24. Complete Loss of Tail Rotor Thrust at Hover PROCEDURE:
    Close throttles immediately and make a hovering autorotation landing.
  25. Complete Loss of Tail Rotor Thrust in Flight PROCEDURE:
    • Close throttles and lower collective pitch immediately. Establish a glide speed slightly above normal autorotation approach speed.
    • If a turn is required to reach a more desirable place to land or to align into the wind, make it to the right.
    • Yaw can be controlled in the following manner:
    • Right Yaw -- a pulse of up collective will produce more friction in the mast thrust bearings, creating a left moment.
    • Left Yaw -- a slight addition of power should arrest it.
    • Complete autorotation.
  26. Fixed Pitch Failure at Hover PROCEDURE:
    Do not close throttles unless a severe right yaw occurs. A landing from a hover can be accomplished with greater safety under power controlled flight rather than by closing throttles and entering autorotation.
  27. Fixed Pitch Failure in Flight, right pedal locked forward of neutral, PROCEDURE:
    • Maintain airspeed at or above 60 knots.
    • Use a shallow to normal approach.
    • At 60 to 75 feet AGL and when the landing area can be made, start a slow deceleration to arrive at the intended landing point with about 25 KIAS.
    • At 2 to 5 feet AGL, slowly reduce throttle to overcome yaw effect and allow helicopter to settle.
    • When aligned with the landing area, allow helicopter to touch down.
  28. Fixed Pitch Failure at Hover, left pedal locked forward of neutral, PROCEDURE:
    • Maintain about 60 knots during initial part of approach.
    • On final, reduce throttles to minimum operating RPM (97% Nr) and simultaneously begin a slow deceleration so as to arrive at a point about 2 feet above the intended touchdown area as ETL is lost.
    • At 2 to 5 feet AGL, use throttle slowly as necessary to maintain alignment with the landing area and to control yaw; do not allow helcopter to settle until alignment is assured, then touch down.
  29. Hydraulic System Failure PROCEDURE:
    • Affected system -- Identify positively.
    • HYDR SYS switch (affected system) -- OFF.
    • NII RPM -- Set to 100%.
    • MASTER CAUTION light -- Reset.
    • Land as soon as possible.
  30. DC Power Failure INDICATIONS:
    • DC GENERATOR caution light illuminates.
    • All lighting and avionics on non-essential buses inoperative.
  31. DC Power Failure PROCEDURE:
    • GEN FIELD and GEN RESET circuit breakers -- Check in.
    • GEN switch (affected generator) -- RESET, then ON.
    • If generator remains inop:
    • GEN switch (affected generator) -- OFF.
    • If No. 2 generator failed:
    • Battery Bus 2 switch -- OFF
    • Battery Bus 1 switch -- ON.
    • NON-ESNTL BUS switch -- MANUAL.
    • DC AMPS -- Monitor: if load exceeds limit:
    • NON-ESNTL BUS switch -- As desired. Switch off unnecessary equip.
    • If both gens fail:
    • Emergency load switch -- As desired.
  32. AC Power Failure INDICATIONS:
    No. 1 or No. 2 INVERTER caution light illumintes.

    Possible loss of power to certain AC instruments (with no INVERTER caution light).
  33. AC Power Failure PROCEDURE:
    • If either INVERTER caution light illuminates, proceed as follows:
    • AC voltmeters -- Check to determine that remaining inverter has assumed all AC loads.
    • INV PWR circuit breakers -- Check in.
    • AP 1 or AP 2 button (affected system) -- Depress to reengage autopilot.
  34. Fuel Boost Pump Failure PROCEDURE:
    • Usable fuel will be approximately 60 pounds less than indicated per failure.
    • If pracitcal descend below 5000' Hp.
    • FUEL INTCON switch -- OPEN.
  35. Fuel Quantity Indications Malfunction PROCEDURE:
    • FUEL QTY circuit breaker -- Recycle (affected side).
    • FUEL INTCON switch -- OPEN.
    • Obtain fuel load by doubling remaining fuel quantity indicated.
  36. AFCS Fails to Engage or Disengages (one autopilot) PROCEDURE:
    • Airspeed -- Reduce to 115 KIAS or less.
    • INV 1 and 2 switches -- ON; check NO. 1 and NO. 2 INVERTER caution lights extinguished.
    • Pilot and copilot ADIs -- Check ATT FAIL flags retracted, indicators functionin properly.
    • Check all circuit breakers in.
